I think the writer decides.

Usually, the writer writes the story in a Doc or a Word thing and shares it with the beta and the beta corrects/gives suggestions

@InkAndMagic, beta reading is where you read someone's fanfic before it is published. You check for many writing errors that the author might have made. You also give them your feedback.
